Spiral Health signed a five-year deal to serve Medicare beneficiaries in Kansas and New Jersey with Clover Health, an innovative technology firm that improves health outcomes for the American elders. The value-based contract provides a tailored multi-modal approach to some of the more vulnerable patients of Clover for Spiral Health.
With the scale of Clover Home Care, its home-based primary care program for medically complex and home-based people, Spiral Health will support the growing number of Clover-aligned recipients with complex chronic diseases with its unique model, including home-based service, virtual care, and remote services, through its new direct contracting scheme, CMS (Centers for Medicare and Medicaid Services).In order to improve treatment compliance, uncover social factors that might influence their health, and help physicians and care teams, the specialized medical team offers eligible people visiting at home, online, or on the telephone.
Within the new CMS Direct Contracting program, Clover will proactively identify Medicare patients with complex chronic conditions eligible for Clover Home Care and who are entitled to the high-touch individualized delivery model of Spiral Health using its proprietary platform, the Clover Assistant. Clover is also proactive. Spiras’ teams of clinical care staff lead by a state-licensed nurse practitioner, respiratory therapists, and other allied healthcare providers collaborate with each patient’s doctors to diagnose, treat, educate and involve the patient in self-care.
